Medicare patients with two or more chronic health conditions are eligible to receive chronic care management from Middle Georgia Heart.
Initial Visit
You’ll meet with your care coordinator for a thorough assessment and informative conversation about your medical history and current health status. It’s important that we understand your health status and journey completely so that we can create a comprehensive, you-centered care plan.
Monthly Check-Ins
Your care coordinator will check in with you to see how you are doing and ensure you get the medical attention you need, whenever you need it.
Solutions
Whether you need your prescriptions refilled or there has been a change in your health that requires a doctor’s assessment, your care coordinator will complete the actions necessary to ensure you receive the right care whenever the need arises.
