Middle Georgia Heart’s remote care monitoring service was created for our patients who need help monitoring their blood pressure or weight. These two factors are vital for health, but don’t necessarily require a trip to the doctor’s office for monitoring. That’s where remote care monitoring comes in! After enrolling in the program, we will send you the equipment you need (the SmartHub device, scale, and/or blood pressure cuff) to monitor your health.
From the comfort of your home, we can monitor your health via this equipment. The data is sent directly to us – not a third party! If we notice anything alarming in the data, we will reach out to you. You will also have a direct phone number for the medical professional in charge of your case.
